Chesterfield High Street Awards Application Form – National / Independent Categories
Congratulations! Your business has been nominated by the public for the National or Independent of the Year category in the Chesterfield High Street Awards.
- One mystery judge will visit your venue (dates to be confirmed, subject to Government guidance, there is no charge for this visit).
- You must also complete this application form by Monday 31st August 2020. If you accept your status as a finalist, but do not complete the application form, you will still remain a finalist, however your chances of winning will be significantly reduced.

- Please use real examples when answering the questions – describe how you do things rather than simply saying the team does it.
